During the 1890s, animal development became associated with glandular activity, with profound implications for pediatric nosology and treatment. The significance of this endocrinological turn of developmental physiology and pathophysiology in part hinges on an often-overlooked continuity with ubiquitous early modern medical thought concerning semen as a recrementitious (reabsorbed) nutrient or stimulant. Mid-19th-century interests in adult sexual physiology were increasingly nerve-centered and antihumoral. Scattered empirical, particularly veterinarian, interests in gonadal developmental functions failed to moderate these explanatory trends. While Brown-Séquard’s rejuvenation experiments (...) still offered no clear starting point for a developmental endocrinology, in 1892 Gaston Variot and Paul Bezançon more explicitly deduced a testicular developmental endocrinological function from various observations on testicular ectopy and a local form of animal “demi-castration.” Ensuing interest in the thyroid, the thymus and in the testicles led to various working conceptions of their respective and putatively reciprocal developmental properties, including the idea of a thyroid–testis axis. From 1896, the pubertal affliction of chlorosis became the subject of multiple opotherapeutic approaches, providing an experimental basis for theories of ovarian internal secretion. Polyglandular therapy, piloted for divergent developmental conditions, remained routine until the 1930s despite the biological inefficacy of many endocrine products. (shrink)

Philosophy of science education can play a vital role in the preparation and professional development of science teachers. In order to fulfill this role a philosophy of science education should be made practical for teachers. First, multiple and inherently incomplete philosophies on the teacher and teaching on what, how and why should be integrated. In this paper we describe our philosophy of science education which is composed of bounded rationalism as a guideline for understanding teachers’ practical reasoning, liberal education underlying (...) the why of teaching, scientific perspectivism as guideline for the what and educational social constructivism as guiding choices about the how of science education. Integration of multiple philosophies into a coherent philosophy of science education is necessary but not sufficient to make it practical for teachers. Philosophies are still formulated at a too abstract level to guide teachers’ practical reasoning. For this purpose, a heuristic model must be developed on an intermediate level of abstraction that will provide teachers with a bridge between these abstract ideas and their specific teaching situation. We have developed and validated such a heuristic model, the CLASS model in order to complement our ASSET approach. We illustrate how science teachers use the ASSET approach and the CLASS model to make choices about the what, the how and the why of science teaching. (shrink)

If a hereditary predisposition to colorectal cancer or breast cancer is diagnosed, most guidelines state that clinical geneticists should request index patients to inform their at-risk relatives about the existence of this condition in their family, thus enabling them to consider presymptomatic genetic testing. Those identified as mutation carriers can undertake strategies to reduce their risk of developing the disease or to facilitate early diagnosis. This procedure of informing relatives through the index patient has been criticised, as it results in (...) relatively few requests for genetic testing, conceivably because a certain number of relatives remain uninformed. This pilot study explored attitudes toward informing family members and relevant practices among clinical geneticists. In general, clinical geneticists consider it to be in the interests of family members to be informed and acknowledge that this goal is not accomplished by current procedures. The reasons given for maintaining present practices despite this include clinical ‘mores’, uncertainty about the legal right of doctors to inform family members themselves, and, importantly, a lack of resources. We discuss these reasons from an ethical point of view and conclude that they are partly uninformed and inconsistent. If informing relatives is considered to be in their best interests, clinical geneticists should consider informing relatives themselves. In the common situation in which index patients do not object to informing relatives, no legal obstacles prevent geneticists from doing so. An evaluation of these findings among professionals may lead to a more active approach in clinical practice. (shrink)

BackgroundCarrier screening is generally performed with the aim of identifying healthy couples at risk of having a child affected with a monogenic disorder to provide them with reproductive options. Expanded carrier screening, which provides the opportunity for multiple conditions to be screened in one test, offers a more cost-effective and comprehensive option than screening for single disorders. However, implementation of ECS at a population level would have implications for genetic counseling practice.MethodsWe conducted semi-structured interviews with sixteen European clinical and molecular (...) geneticists with expertise in carrier screening to explore their views on the implementation of ECS in the clinical setting.ResultsUsing inductive content analysis, we identified content categories relevant to the pre- and post-test settings. Participants believed ECS would ideally be targeted at couples before pregnancy. There was some disagreement regarding the acceptability of performing ECS in individuals, with several participants actively opposing individual-based screening. In addition, participants discussed the importance of ensuring informed and voluntary participation in ECS, recommending measures to minimize external pressure on prospective parents to undergo testing. A need for adequate counseling to foster informed, autonomous reproductive decision-making and provide support for couples found to be at risk was emphasized.ConclusionsPractical challenges in optimizing pre-test education and post-test counseling should not be underestimated and they should be carefully addressed before implementing ECS in the clinical setting. Background: Discussing treatment risks has become increasingly important in medical communication. Still, despite regulations, physicians must decide how much and what kind of information to present. Objective: To investigate patients’ preference for information about a small risk of a complication of colonoscopy, and whether medical and personal factors contribute to such preference. To propose a disclosure policy related to our results. Design: Vignettes study. Setting: Department of Gastroenterology, Academic Medical Centre, the Netherlands. Patients: 810 consecutive colonoscopy patients. Intervention: A home-sent (...) questionnaire containing three vignettes. Vignettes varied in the indication for colonoscopy, complication severity and level of risk. Patients were invited to indicate their wish to be informed and the importance of such information. In addition, sociodemograhic, illness-related and psychological characteristics were assessed. Main outcome measurements: Wish to be informed and importance of information. Results: Of 810 questionnaires, 68% were returned. Patients generally wished to be informed about low-risk complications, regardless of the indication for colonoscopy or the severity of the complication. The level of risk did matter, though (OR = 2.48, SE = 0.28, p = 0.001). The information was considered less important if done for population screening purposes or diagnosis of colon cancer, if the complication was less severe (bleeding) and if the risk was smaller (0.01% and 0.1%). Patients’ information preference was also related to age, mood and coping style. Limitations: Difficulty of vignettes. Conclusions: Patients generally wish to be informed about all possible risks. However, this might become uninformative. A stepwise approach is suggested. (shrink)

BackgroundIn the Canadian Alliance for Healthy Hearts and Minds (CAHHM) cohort, participants underwent magnetic resonance imaging (MRI) of the brain, heart, and abdomen, that generated incidental findings (IFs). The approach to managing these unexpected results remain a complex issue. Our objectives were to describe the CAHHM policy for the management of IFs, to understand the impact of disclosing IFs to healthy research participants, and to reflect on the ethical obligations of researchers in future MRI studies.MethodsBetween 2013 and 2019, 8252 participants (...) (mean age 58 ± 9 years, 54% women) were recruited with a follow-up questionnaire administered to 909 participants (40% response rate) at 1-year. The CAHHM policy followed a restricted approach, whereby routine feedback on IFs was not provided. Only IFs of severe structural abnormalities were reported.ResultsSevere structural abnormalities occurred in 8.3% (95% confidence interval 7.7–8.9%) of participants, with the highest proportions found in the brain (4.2%) and abdomen (3.1%). The majority of participants (97%) informed of an IF reported no change in quality of life, with 3% of participants reporting that the knowledge of an IF negatively impacted their quality of life. Furthermore, 50% reported increased stress in learning about an IF, and in 95%, the discovery of an IF did not adversely impact his/her life insurance policy. Most participants (90%) would enrol in the study again and perceived the MRI scan to be beneficial, regardless of whether they were informed of IFs. While the implications of a restricted approach to IF management was perceived to be mostly positive, a degree of diagnostic misconception was present amongst participants, indicating the importance of a more thorough consent process to support participant autonomy.ConclusionThe management of IFs from research MRI scans remain a challenging issue, as participants may experience stress and a reduced quality of life when IFs are disclosed. The restricted approach to IF management in CAHHM demonstrated a fair fulfillment of the overarching ethical principles of respect for autonomy, concern for wellbeing, and justice. That Ibn Sīnā’s “Canon of medicine” figures among the major classics of the history of medicine is doubted by no serious historian of medicine, eastern or western, Islamic or non-Islamic alike. It is therefore all the more surprising that so far no serious critical edition of this text was available. Certainly, a first, very timid step toward a really critical edition was made at the Institute of the History of Medicine and Medical Research, under the direction of Hakeem Abdul Hameed. (...) It compared the four existing editions: Rome 1593; Būlāq 1877; Tehran 1878; and Lucknow 1905. In addition it used an ancient manuscript of Aya Sophia, dated 618, i. e. MS Aya Sophia 3686. With this new edition a further important step toward a full critical edition is made. Even if it is obvious that it does not yet present a “critical edition” in the full sense of the word, it has important merits. (shrink)

In the present paper it is shown that al-Ämidî has tried to integrate somehow pilosophy into theology in way that reveals closer to al-Ghazâlî's than to F.D. al-Râzî's way of combining both currents of thought. Based on an analysis of several ideas taken from different works it is shown that al-Ämidî not only accepts the logic of the philosophers, but also their views that may be considered as scientifically established beyond any reasonable doubt.

Bringing continental sensibilities and skill to his project, David Janssens has abandoned the line of defense heretofore used by North American intellectuals to shield Leo Strauss from criticism: Janssens wastes no time trying to prove Strauss was a liberal democrat, frankly admits his atheism, and emphasizes the continuity and European origins of his thought. Nevertheless committed to defending Strauss even at his most vulnerable points, Janssens is compelled to anchor his new defensive position on a misreading of what he calls (...) This collection of 17 articles offers an overview of the philosophical activities of a group of philosophers (who have been) working at the Groningen University. The meta-methodological assumption which unifies the research of this group, holds that there is a way to do philosophy which is a middle course between abstract normative philosophy of science and descriptive social studies of science. On the one hand it is argued with social studies of science that philosophy should take notice of what scientists (...) actually do. On the other hand, however, it is claimed that philosophy can and should aim to reveal cognitive patterns in the processes and products of scientific and common sense knowledge. Since it is thought that those patterns can function as guidelines in new research and/or in research in other disciplines, philosophy can nevertheless hold on to the normative aim which is characteristic of 'classical' philosophy of science. Compared to this common assumption, there is a diversity of subjects. Some papers deal with general problems of science, knowledge, cognition and argumentation, others with topics relating to foundational problems of particular sciences. Therefore this volume is of interest to philosophers of science, to philosophers of knowledge and argumentation in general, to philosophers of mind, as well as for scientists working in the physical and applied sciences, biology, psychology and economy who are interested in the foundations of their disciplines. In this paper I build a case for considering the pioneering behaviourist philosopher Grace de Laguna as one of the grandmothers of analytic philosophy. I argue against the ‘Great Men’ narrative of analytic philosophy as composed of Moore, Russell, Wittgenstein and their followers, and in favour of a more inclusive ‘movement’ narrative of analytic philosophy as a broad and varied movement with an anti-idealist and naturalistic orientation aimed at fitting around novel development in the sciences, including Einsteinian physics and psychology. I compare de Laguna’s prescient early views to Whitehead’s; both share an anti-idealist but also anti-atomist solution to the problem of perception, and both oppose the bifurcation of nature, with de Laguna expressing these views before Whitehead did. I conclude that while both opposed the narrower project of logical atomism, both count as ancestors of the wider analytic movement. (shrink)

Quantum Structures and the Nature of Reality is a collection of papers written for an interdisciplinary audience about the quantum structure research within the International Quantum Structures Association. The advent of quantum mechanics has changed our scientific worldview in a fundamental way. Many popular and semi-popular books have been published about the paradoxical aspects of quantum mechanics. Usually, however, these reflections find their origin in the standard views on quantum mechanics, most of all the wave-particle duality picture. Contrary to relativity (...) theory, where the meaning of its revolutionary ideas was linked from the start with deep structural changes in the geometrical nature of our world, the deep structural changes about the nature of our reality that are indicated by quantum mechanics cannot be traced within the standard formulation. The study of the structure of quantum theory, its logical content, its axiomatic foundation, has been motivated primarily by the search for their structural changes. Due to the high mathematical sophistication of this quantum structure research, no books have been published which try to explain the recent results for an interdisciplinary audience. This book tries to fill this gap by collecting contributions from some of the main researchers in the field. They reveal the steps that have been taken towards a deeper structural understanding of quantum theory. (shrink)

How do people interleave attention when multitasking? One dominant account is that the completion of a subtask serves as a cue to switch tasks. But what happens if switching solely at subtask boundaries led to poor performance? We report a study in which participants manually dialed a UK-style telephone number while driving a simulated vehicle. If the driver were to exclusively return his or her attention to driving after completing a subtask (i.e., using the single break in the xxxxx-xxxxxx representational (...) structure of the number), then we would expect to see a relatively poor driving performance. In contrast, our results show that drivers choose to return attention to steering control before the natural subtask boundary. A computational modeling analysis shows that drivers had to adopt this strategy to meet the required performance objective of maintaining an acceptable lateral position in the road while dialing. Taken together these results support the idea that people can strategically control the allocation of attention in multitask settings to meet specific performance criteria. (shrink)

An odd dissensus between confident metaphysicians and neopragmatist antimetaphysicians pervades early twenty-first century analytic philosophy. Each faction is convinced their side has won the day, but both are mistaken about the philosophical legacy of the twentieth century. More historical awareness is needed to overcome the current dissensus. Lewis and his possible-world system are lionised by metaphysicians; Quine’s pragmatist scruples about heavy-duty metaphysics inspire antimetaphysicians. But Lewis developed his system under the influence of his teacher Quine, inheriting from him his empiricism, (...) his physicalism, his metaontology, and, I will show in this paper, also his Humeanism. Using published as well as never-before-seen unpublished sources, I will make apparent that both heavy-duty metaphysicians and neopragmatist antimetaphysicians are wrong about the roles Quine and Lewis played in the development of twentieth-century philosophy. The two are much more alike than is commonly supposed, and Quine much more instrumental to the pedigree of current metaphysics. (shrink)

Abstract:The concept of competency (or competence) entered educational policy by the end of last century. Due to this concept’s background in recruitment and because of its generic character, many definitions have been given, but so far no unified, transparent, and consistent definition has come forward. The concept of competency has also reached art education. Due to the “artistic” character of this domain of learning, this concept seems to be a powerful instrument to describe the types of behavior and learning goals (...) that are typical for this domain and for the related ways of thinking, working, and learning. This article gives a critical overview of what is seen as typical for what competency refers to in the domain of (art) education. A new definition of competency is presented that covers both the productive and the receptive domains of learning in visual art education. The constituent elements of this definition are discussed in detail. The analysis is broadened to include aspects of human cognition and behavior that can be considered as typical for visual art education, such as the aesthetic experience and its moral dimension. Pour l'édition de la peine première partie traduite à Tolède, qui couvre le prologue et le début du premier chapitre, les mêmes principes ont été maintenus que pour l'édition du second traité. Quant à l'édition de la majeure partie, qui fut traduite à Burgos quasiment un siècle après la tolédane et qui n'a été conservée que dans un seul témoin, (...) les cas où la traduction latine ne correspond pas à la lecture du texte arabe sont analysés de façon particulière et, autant que possible, expliqués sur la base de considérations philologiques et/ou doctrinales. En outre, dans la mesure du possible, les sources historiques des doctrines évoquées par Avicenne dans l'ensemble de ces chapitres ont été précisés. (shrink)

The title of this collection of essays reflects the confusing complexity of the issues at stake. “Arts,” “Beyond Art,” and “Art”: what “art” are we talking about? In the introductory chapter, the editors follow, among others, the analyses of Arthur Danto and Pierre Bourdieu, concluding that “art,” as a series of canonical art works that reflect and legitimize the taste of an elite and defines the content of arts education, has come to an end. More specifically, the focus should change (...) from the qualities of works of art to the processes of imaginative consciousness and to other cognitive processes that give form to the “artistic” as a dimension of life. The use of coercive measures in the care for the addicted has changed over the past 20 years. Laws that have adopted the “dangerousness” criterion in order to secure patients’ rights to non-intervention are increasingly subjected to critique as many authors plead for wider dangerousness criteria. One of the most salient moral issues at stake is whether addicts who are at risk of causing danger to themselves should be involuntarily admitted and/or treated. In this article, it is argued that the (...) dilemma between coercion on the one hand and abandonment on the other cannot be analysed without differentiated perspectives on the key notions that are used in these debates. The ambiguity these notions carry within care practice indicates that the conflict between the prevention of danger and respect for autonomy is not as sharp as the legal systems seem to imply. Some coercive measures need not be interpreted as an infringement of autonomy—rather, they should be interpreted as a way to provide good care. (shrink)
